Please read these guidelines carefully before completing your application form. 
PART A: GENERAL GUIDELINES FOR GRANT SUBMISSION 
INTRODUCTION
The Win the Day Organisation is a public, non-profit organisation established primarily to financially support families with accommodation, meal assistance and one-off treatment grants, as well as to raise awareness and support the funding of research related to rare childhood cancers.

AIM AND AVAILABLE FUNDING
The aim of the Win The Day Family Assistance Funding, is to support families fighting childhood cancers with: 
·        Funding for out-of-pocket accommodation costs (up to $5,000 per year). 
·        Meal assistance during hospital stays. 
·        A one-off payment for Treatment Support for families a recent diagnosis of a childhood cancer relapse (up to $2,000). 

Our available funding relies on the generosity of our supporters, sponsors and donations. If funds have been exhausted, families will be placed on a waiting list for up to 3 months upon receiving their application. After 3 months, the Win the Day Funding Committee will make contact to see if you would like to stay on the waiting list.
ELIGIBILITY CRITERIA
Only families that meet all of the following eligibility requirements may apply:
-      Your child is under 15 years of age, (if the child is 15-18 years of age, it will be up to the discretion of the grant assessment team) 
-      Your child has a diagnosis of a childhood cancer (please see the section below on Broad Categories of Rare Children's Cancers as they will be prioritised)
-      Live in the Australia and are required to travel further than 100km to your specialised oncology hospital (will need to provide photo ID or utility bill showing current address)
-      You have a support letter from your Social Worker, practicing Oncology Doctor or General Practitioner (please see Supporting Evidence section below for requirements).

In addition:
-      The one-off payment for Treatment Support is specifically to support those families that have had a recent diagnosis of a childhood cancer and has relapsed or the treatment is ongoing (more than 3 months of treatment)
-       Applications for the out-of-pocket accommodation cost will need to provide evidence that they have applied for the IPTAAS Scheme (or something similar) for travel and accommodation assistance (please see Supporting Evidence section below for requirements). You will also need to supply Win the Day Charity with an invoice for the accommodation sourced so we can pay the accommodation bill.
BROAD CATEGORIES OF RARE CHILDREN'S CANCERS
Children’s Liver Tumours 
·        Hepatoblastoma (usually occurs in children under five) 
·        Hepatocellular carcinoma or HCC (which is rarer and usually occurs in older children) 
·        Liver cancer can either be primary (starting in the liver) or secondary (metastatic and spread from another part of the body). 

Other Rare Childhood Cancers 
Tumours that normally only occur in adults are occasionally found in children and includer cancers of the digestive system, the thyroid and the adrenal gland. Rare tumours which only occur in children include: 
·        Pancreatoblastoma 
·        Malignant rhabdoid tumours 
·        Melanotic neuroectodermal tumours of infancy 
·        Rare tumours in the head and neck area 
·        Rare hormonal/endocrine tumours (Phaeochromocytoma) 
·        Rare brain tumours (Meningioma)
·        Rare skin tumours 
SUPPORTING EVIDENCE
The Win the day Organisation will accept grant applications on the proviso the family have provided a support letter from their Social Worker, practicing Oncology Doctor or General Practitioner. In this letter it needs to state the following: 
-       Full name of child
-       Date of birth of child 
-       Date of diagnosis 
-       Estimated length of treatment 
-       Type of Cancer 
-       Treating Hospital 
-       Brief description of the family situation

Applications for assistance with accommodation will need to supply evidence that they have applied for the IPTAAS Scheme for travel and accommodation assistance. The Win the Day Grant will reimburse the remaining out of pocket cost the family may incur up to the value of $5,000 per year. FURTHER INFORMATION ON EACH GRANT TYPE
Accommodation Grant

Families will be supported with funding to reimburse the remaining out of pocket costs the family may incur up to the value of $5,000 per year (see above for further requirements on the IPTAAS Scheme). Families will be supported up to 6 months after completing treatment for the accommodation grant.

One-off Treatment Support Grant
The one-off treatment support grant is a one-off payment for a family that have had a recent diagnosis of a childhood cancer and has relapsed or has been ongoing treatment for more than  6months of treatment. The amount is a one-off payment of $2,000 to be used to alleviate financial stresses the family may be experiencing from the recent diagnosis that has exceeded 6 months. 
ASSESSMENT PROCESS
The Win the Day Organisation will review each application against the eligibility criteria as it is received. Please ensure you have completed the Checklist in the application form, as the review of your eligibility may be delayed if any information is missing.

All eligible application will be assessed and prioritised on the following:
1.      Your application will be placed in a list in order of when your application was received (or if information was missing, after all required information has been supplied and the application is deemed eligible) 
2.      Where funds are available, we aim to approve the full amount of grants and services applied for. 
3.      If funds are insufficient to meet demand, we would like to provide some support to all eligible families, where possible. In this circumstance, we will consider the prioritisation of grant type that you provided in criteria 1 of your application. 
4.      If funds have been exhausted, families will be placed on a waiting list for up to 6 months upon receiving their application. 

All families will be contacted via email within 7 days of submitting their application. We aim to advise families of the outcome of their application at this time, or as soon as possible thereafter.

Please provide us with a detailed overview of your child’s exact diagnosis, and prognosis (if known), as well as what current treatment plan your child is undertaking.

We would like to invite you to share your family's story. Help us provide the broader community with information on how they might be able to help families affected by rare childhood cancer.
